ServicesHoyt Nursing & Rehab Centre is the place where Saginaw-area citizens often turn for post-operative care or to recover from illnesses such as strokes and fractures. We are also a leading local provider of long-term elder care. The Hoyt staff uses a person-centered planning process which involves families, friends and professionals as the resident desires or requires. Lodging and AccommodationsResident rooms are arranged for private or semi-private occupancy. Furnished rooms are set up to meet individual needs while respecting the rights of any roommates. A typical room includes an adjustable bed with linens, bedside stand, dresser, closet, and a bathroom. All rooms have large windows making them bright and cheery. Residents are encouraged to bring in family photos and other items to personalize their space. |

DiningResidents love our home-cooked foods served restaurant-style. Residents may choose from a menu of two well-balanced entrées which include choice of vegetable, fruit, bread, beverage, dessert, plus salads, soups, and sandwiches upon request. Each resident is provided meals and supplements that meet their individual dietary preferences and therapeutic needs according to our in-house Dietician’s assessment and physician orders. Residents’ favorite dishes include barbequed ribs, steak, and shrimp. During the summer, the sweet aroma of outdoor grilling draws residents outside to the patio where our dietary staff can be found flipping America’s favorites—from hot dogs and hamburgers to chicken and steak. At holiday time, we go “all out” to prepare memorable meals that are served by staff volunteers with family members invited free of charge. At any other time, friends and family members are welcome to join residents for meals for a nominal fee. In addition, we offer daily “Coffee and Cheer” at Noon, seven days a week, and also at 5 p.m. on weekdays. |

PhysiciansDr. Iriana Hammel, M.D. is our Medical Director. She oversees physician services and employee health. As a consultant to the nursing leadership team, she offers guidance on policies and procedures that relate to medical and nursing services. At a minimum, doctors visit each resident every 30 days for the first 90 days that they are at the facility, and every 60 days thereafter. In addition, certain doctors are available on-site weekly. Doctor visits can also be requested anytime by Hoyt nursing staff, residents or resident family members. |

Quality Nursing CareThe nursing department is led by the Director of Nursing Services, Julie Carpenter RN and the Nursing Leadership Team. Together they ensure that each resident is properly assessed for the care they need and that their record reflects their response and progress in meeting their care goals. The nursing department provides high quality care with a positive, team-oriented approach. They are open to a collaborative sharing of ideas with all team members, disciplines, families and vendors in an effort to promote resident well-being, health and quality of life. |

Rehabilitation TherapyHoyt is not only an outstanding skilled nursing facility, but also offers excellent rehabilitation services. The Rehabilitation department, headed by Dan Peterson, Registered Physical Therapist, consists of a staff providing physical, speech and occupational therapies. For many residents, their goal is to return to a more independent living situation. We all work together to make this a reality. |

Restorative TherapyAfter the therapy department discharges residents from their care, a lead restorative nurse and nurse aides take over. They facilitate regular activities including the popular “Restorative Ball” group, walking programs, range-of-motion exercises, and individual care plans—all intended to maintain residents’ strength and mobility—in a fun, social setting.

Housekeeping and LaundryThe clean, comfortable surroundings enjoyed by our residents and staff is the responsibility the Director of Environmental Services, who heads the departments of Laundry, Housekeeping, Maintenance, and Janitorial. These departments ensure the general upkeep, cleaning and laundering of the facility, equipment, linens and residents’ personal items. Some of the employees from these departments are known for their “gift of gab” and provide daily social contact with residents while providing these services.
Activities and Community OutingsTherapeutic and interesting activities are scheduled and available seven days a week, throughout the day, as well as in the evening. HospiceHeartland Hospice is here to help residents and their loved ones live life to the fullest, no matter how much time is left. Heartland's staff of professionals manages the resident's end of life journey. The hospice program is designed to meet each resident's choices and goals. Hospice services include nurses, nurse aides, chaplains and social services. For more information, ask to speak with a Hoyt nursing staff member. They will arrange a meeting with Heartland Hospice. Additional ServicesThe services listed below are available to residents at an additional cost via outside providers. Some of these services may require a physician order or referral.

Social Services
Residents’ emotional and spiritual well-being is often just as important as their physical health. Hoyt’s Social Services team ensures that these needs are met through direct care and counseling, as well as through coordinated community and family services. When a resident requires special care that cannot be met by the facility’s services, the Social Services team works with the resident and family to meet those needs.
